Signs аnd symptоms оf а pulmоnаry embolism, which include shortness of breath, chest pain, tachycardia, and hypoxia. Patients may also have signs of lower extremity DVT, such as leg swelling and tenderness, since most pulmonary thrombi originate as emboli from the iliofemoral region. Because the symptoms of PE are nonspecific, one must have a high suspicion of it, especially in patients who exhibit the criteria outlined by Virchow’s triad. For example, this patient has likely had prolonged immobility due to his recent large surgery. In addition, his malignancy has likely caused him to be in a hypercoagulable state. This patient needs CT angiography of the chest using a PE protocol to diagnose the arterial clot. However, if suspicion is high enough given the patient’s history, signs and symptoms, one should not delay treatment. The treatment for a DVT or PE is usually anticoagulation with an intravenous heparin drip.

The pаtient with diаgnоsis оf Lung cаncer  lung cancer. Central lоcation and tendency to exfoliate malignant cells into sputum are more common with squamous cell carcinomas of the lung, which are non-small cell lung cancers. Asbestos exposure is associated with an increased risk of lung cancer, pulmonary fibrosis, and malignant pleural mesothelioma. In patients with a central lesion suggestive of lung cancer, sputum cytology is the most appropriate next step. Bronchoscopy is the procedure of choice for obtaining the tissue diagnosis and has a sensitivity of 88% for the diagnosis. For a tumor with submucosal spread or peribronchial tumor with extrinsic compression of bronchus, the addition of transbronchial needle aspiration with endobronchial ultrasound further increases sensitivity. Computed tomography scan of the chest and upper abdomen is indicated for the staging of non-small cell lung cancer in a newly diagnosed patient because it can detect metastasis to liver and adrenal glands. Magnetic resonance imaging (MRI) of the thorax is not indicated in the diagnostic evaluation of suspected non-small cell lung cancer. MRI is more appropriate for the staging of non-small cell lung cancer. Pleural biopsy is indicated in patients with suspected pleural malignancy when pleural fluid cytology findings are negative. Positron emission tomography scan plays an important role in the staging of non-small cell lung cancer. It is not useful in the diagnosis of lung cancer.
