Patient Name: Santiago, Juan DOB: August 27, 1945 Pre-Operative Diagnosis: Degenerative joint disease Post-Operative Diagnosis: Osteoarthritis, left hip Procedure Performed: Left THA (Total Hip Arthroplasty) Anesthesia: Both spinal and epidural used Complications: None Description of Procedure: The patient was taken to the operating room and placed in the supine position after the adequate administration of anesthesia. The left hip was prepped and draped in the usual sterile fashion. A longitudinal incision was made over the lateral aspect of the proximal thigh and centered over the greater trochanter while the subcutaneous tissues were divided sharply. Hemostasis was achieved by means of electrocautery. The iliotibial band was divided in the line with its fibers. The greater trochanteric bursa was exposed and excised. A lateral gluteus medius splitting direct lateral approach was performed, and the femoral head was dislocated. Advanced degenerative changes were seen. The head and neck were amputated and saved for pathologic analysis. Attention was then turned to the acetabulum which was denuded of articular cartilage and progressively reamed to a hemispherical shape. A 49 mm acetabular shell was found to fit securely and was impacted into a position after the placement of bone graft. Acetabular screws and ceramic head were used. Attention was then turned to the femoral canal, which was broached and sounded in the standard fashion. A size 14 extended lateral offset broach with a 36-mm head and a 3- mm neck was found to provide optimal restoration of leg length, stability, and motion. After further bone preparation, the actual components were impacted, and the hip was reduced. The wound was closed after pulsatile saline irrigation using #1 Monocryl capsular, gluteus medius, vastus lateralis and fascia lata sutures, #2-0 Monocryl subcutaneous sutures and skin staples completed the closure. Compressive dressings were applied. An abduction pillow was placed between the lower extremities to ensure hip stability.
