Answer оne оf the fоllowing questions only Scenаrio 1 Gаry is аn unemployed single man.  He is 28 years old.  He lives with his grandparents.  He does not have children and is not currently in a relationship.  He has a diagnosis of schizophrenia.  Gary reports ongoing adherence to his treatment plan.  He is prescribed olanzapine and mirtazapine.    Gary is a smoker.  He drinks alcohol.  He smokes rolling tobacco.  He drinks alcohol in binges.  He estimates 45 units of strong cider (7.5% alcohol) once a week.  Gary feels he is “sleeping too much”.  He reports an increased appetite.  He had an admission to an acute psychiatric inpatient unit six months ago.  This admission was triggered by the death of his father.  Gary reports his father had “heart problems”.  His father had significant mental health problems but is unsure of their nature.  He has occasional contact with his mother and sister who also live locally.  It was noted at this time that Gary was misusing diazepam.  Gary denies current misuse of drugs or prescription medication. He started his use of olanzapine while an inpatient.  Since then he has gained 2 stone in weight and Gary attributes this to several factors including medication.  He is 6’1’’ and 17 stone 7 lbs. When well Gary likes to walk to the local park and spend time there but recently his exercise tolerance has dropped and he struggles to walk to the local shop and back without becoming breathless. He has also expressed a sensitive of gripping in his chest which eases off when he gets his breath back but it causes his stress.   Gary enjoys spending time with his sister.  He likes playing video games and watching films on television. Gary may become withdrawn and isolative when unwell.  He concedes he does shout and argue more with his grandparents when he is unwell.  He denies any risk history of harm to others.  He has indicated some occasional thoughts of hopelessness but denies any thoughts or planning for suicide.      Provide a summary of the altered physiology of a patient suffering with ischaemic heart disease and associated angina. Include in your discussion how the normal heart function is compromised as well as the other symptoms a patient like Gary might experience and potential treatments for his condition (10 marks)  Using a suitable framework and the information provided construct a detailed formulation of the problems identified.  Discuss the risk factors that may also be present and how you would confirm these. (10 marks)  Describe the outcomes you may draw from nursing assessment.  Identify potential needs and interventions that will be suitable. (10 marks)  Discuss a potential health promotion initiative Gary could engage in to improve his physical health and include what the benefits of this initiative could have on NHS resources and capacity also? (10 marks)    Scenario 2 Sarah is a 47 year old woman who has recently been diagnosed with bowel cancer following investigations by her GP. She attended her GP with symptoms of abdominal pain and a change to her bowel pattern. She has been reviewed by the surgical team who hope to be able to remove her tumour but have made her aware depending on how the operation goes and the characteristics of her tumour she may need to have chemotherapy. Sarah is a mother of two children who are 16 and 18. They both live at home but her son (age 18) is about to leave for University in another city. She works part time as a paralegal at a law firm and her husband works in the oil industry is frequently required to travel for work. Sarah leads a reasonably healthy lifestyle, attending a fitness class twice a week with her friends and walking the family dog. She eats a balanced diet although sometimes skips dinner when her husband is away and her children are elsewhere for dinner. She has a past medical history of post natal depression following the birth of her second child and for a time was taking citalopram 10mg but this was discontinued some years ago. Sarah does like a glass of wine at the weekends and is worried this has caused her cancer. Sarah is an ex-smoker of 5 years having previously smoked 20 cigarettes per day (although not during her pregnancies). She was pleased to stop although the stress of her recent diagnosis has meant she has smoked on a couple of occasions. Her family are frustrated about this as her son has read that chemotherapy works less well in people who smoke.   Discuss the pathophysiological changes that happen in a patient with cancer. Your answer should include what risk factors increase a person’s chance of developing cancer and a discussion of the cellular changes that happen when a tumour forms (10 points) Identify three potential care needs Sarah has, discuss the potential impact on her and identify a potential solution to each need (10 points) Identify and discuss the potential psychological and social needs Sarah is likely to have as a result of her cancer diagnosis and include some potential care plans and support services that could help meet these needs. Your answer should include the potential impact on Sarah and her family unit if these needs go unmet. (10 points) Select and discuss a public awareness campaign currently used for cancer. This does not need to be related to bowel cancer. Your answer should include the detail of the campaign, who is the target audience, what the campaign is aiming to achieve and what the positive implications on health service in relation to cost and capacity are when a health campaign is successful. (10 marks)
