He has recently been referred to the school psychologist (you) by his homeroom teacher, who has reported that Toby’s behaviors have begun to impact his academic performance. Specifically, Toby’s teacher reported that he often engages in interfering behaviors that are inconsistent with behavioral expectations in the classroom and are beginning to impact the academic environment. Specifically, she reports that Toby is frequently out of his seat, attempting to get the attention of his peers, and using inappropriate language. He also sometimes leaves the classroom without permission, roaming the halls of the school building until the behavior specialist is called to escort him back to his classroom. These behaviors are preventing him from completing his in-class assignments. He has also been failing to complete his homework or get his planner signed by a caregiver. His teacher would like you to follow up on his academic and behavioral issues.   Since this is your first year at Tampa Elementary School you are not very familiar with Toby but you know that he was identified as a special education student under the classification of Other Health Impairment last year. You were not involved in the initial evaluation process. A review of Toby’s cumulative folder reveals that his grades are lower compared to previous years. Specifically, he has mostly Cs across subjects with the exception of a D in science and a B in language arts. Toby earned all A’s and B’s in 1st grade but his grades have declined steadily since. Additionally, his latest standardized academic assessments reveal that he is performing well below grade level expectations in reading and mathematics. Toby also has a record of office discipline referrals since he started at Tampa Elementary School for behaviors consistent with his current teacher’s report. A review of Toby’s IEP reveals that he receives supplementary instruction in reading and math in the form of pull-out groups but is otherwise educated in the general education setting. His IEP also contains a behavioral goal related to his classroom behavior. The goal states that “Toby will meet classroom behavioral expectations 4/5 (80%) days a week. The goal is supported by 30min/week of counseling support. In a brief conversation with the school counselor at Tampa Elementary School you learn that these meetings have been relatively unstructured with no data to support decision-making regarding their effectiveness. There is a behavior plan attached to the IEP that was generated by a web-based application the district recently purchased that promises “legally-defensible behavior plans in under an hour.” The plan states that Toby’s teacher should use “I statements,” “Extinction” and “Reinforcement” to address his interfering behaviors and increase academic engagement behavior. The level of detail is insufficient for implementation and there is no evident data or progress monitoring plan.   You decide to meet with Toby as a follow-up on his teacher’s referral. In your meeting with Toby he reports having difficulty concentrating and staying focused at school. He states that he feels lost easily, especially when he doesn’t know how to do the work. He reported that he leaves the classroom when he feels this way because his teacher doesn’t give him the help he needs. He reports enjoying talking to the behavior specialist and the school counselor.   Toby’s mother reports that he has had a medical diagnosis Attention Deficit/Hyperactivity Disorder (ADHD), Combined Type, which he received last year. He is not prescribed any pharmaceutical treatment at this time. His mother reports seeing behaviors similar to what school staff are reporting in community settings (e.g., library) and that Toby has had difficulty managing his own behavior from an early age. Toby’s mother works during the evening and is not at home when he is supposed to be doing his homework and getting his planner signed. Toby’s father is incarcerated and has not had any contact with Toby since he was 2 years old. Toby’s older brother (16yo) arrives home from high school around the same time as Toby and is responsible for Toby’s care in the afternoons and evenings. Toby has no other siblings but spends much of his free time outside playing with other children in the neighborhood.   In your role at Tampa Elementary School, you have the opportunity to support Toby, his teaching staff, and his parents. Your response today should consider the affective, biological, cognitive, developmental, and social aspects of behavior relevant to conceptualizing Toby’s current behavior (hypotheses developed in Day 1), and ultimately supporting Toby directly through clinical services. Today, you need to decide how to treat Toby through comprehensive school-based psychotherapy.
