Impression: Pneumonia                  Orders:  Administer STAT intravenous bolus of 250 ml of 0.9% Sodium Chloride If MAP is less than 65 after the completion of the first fluid bolus, administer a second intravenous bolus of 250 ml of 0.9% Sodium Chloride. Initiate Sepsis Protocol: Measure lactate level* * Remeasure lactate if initial lactate is elevated (> 2 mmol/L). Obtain blood cultures before administering antibiotics. Administer broad-spectrum antibiotics. Administer Vancomycin 1 gram  intravenously NOW Begin rapid administration of 30mL/kg crystalloid for hypotension or lactate level ≥ 4 mmol/L. Apply vasopressors if hypotensive during or after fluid resuscitation to maintain MAP ≥ 65 mmHg.
