JK is а 72 yeаr оld mаle whо has transitiоned from his apartment to an assisted living facility.  His medical history includes hypertension, high cholesterol, enlarged prostate, chest pain, heartburn, and arthritis.  His current medications are Lisinopril/HCTZ (hydrochlorothiazide), Lasix, Simvastatin, Hytrin, Prilosec, nitro patch, and glucosamine.  He is 5'9" and weighs 154 lbs.  His usual weight in his admission notes is 163 lbs. Because JK had missed some of his usual activities, the nurse on staff goes to visit him.  Upon arrival she finds that he is not feeling well.  He complains that he has been constipated for several days. Today he notes that his vision is blurry, and he reports his mouth is very dry and he feels hotter than usual.  Upon further questioning, the nurse learns that Joe has been experiencing an increase in nighttime urination so he has stopped drinking any fluids with dinner and in the evening.  JK is now getting up just once a night but now his urine smells funny.  HIs recent labs were: Labs:                           Normal       Units                  JK's results              Albumin                     3.5-5           g/dl                    3.9                                  Total Protein             6-8              g/dl                     7.2                                  Sodium                      136-145      mEq/L               148                                Potassium               3.5-5.5        mEq/L                   3.2                                  Chloride                  95-105        mEq/L                  105                                Magnesium            1.8-3            mg/dl                     2.2                                   Osmolality              285-295      mmol/kg/H2O     309                                Glucose                   70-110        mg/dl                     91                                        BUN                         8-18             mg/dl                    22                                  Creatinine               0.6-1.2        mg/dl                     1.4                                   Calcium                   9-11             mg/dl                     9.1      Urinalysis:                Normal                                                  JK"s levels Color                         pale yellow                                              amber                             Appear                     clear                                                          cloudy                             Sp gravity                 1.003-1.030                                             1.041                               pH                              5-7                                                            4.8                                    Glucose                    Neg                                                            Neg                                Ketones                    Neg                                                              +1                                     WBCs                         0-5                                                               0                                       RBCs                           0-5                                                               0                                        JK reports he eats his breakfast in his room.  He generally makes coffee, and may have some cereal with 2% milk or toast with butter.  He eats lunch in the dining room.  He gets whatever the main meal is that consists of a protein, starch and vegetable.  He usually drinks another cup of coffee either with or after his meal.  In the afternoon, he likes to have a beer.  His dinner may be in the dining room (another plate of the main meal) or a can of soup in his room.  He does not drink any fluids with dinner.  He seldom snacks.                            1.  What signs does JK exhibit that indicate he may be dehydrated?  What blood and urine labs help confirm this?  (8 pts) 2.  Calculate JK's fluid needs.  Estimate how well he is meeting his needs.  (3 pts) 3.  Given the medications he takes, which one might be contributing to his dehydration?  State two (2) drug/nutrient interactions you would need to counsel him about this medicaiton.  (4 pts) 4.  Write 2 questions you would like to ask JK.
