Outcоmes-Bаsed Cоntrаct: The drug pricing wаtchdоg ICER (Institute for Clinical and Economic Review) conducted a clinical and economic review of Zynteglo before it was launched. ICER assumed a launch price of $2.1 million and concluded that Zynteglo was cost-effective using traditional cost-effectiveness methods and thresholds. Since Zynteglo is a one-time gene therapy with potentially a lifetime of cost off-sets, ICER conducted a second model where half of the lifetime cost savings from Zynteglo was returned to society rather than being credited to the price of the treatment. In this model, discounts between 15% and 38% off the assumed launch price of $2.1 million were needed to meet commonly accepted thresholds. “ICER-Beta Thalassemia Report at a Glance” In August 2022, bluebird bio launched Zynteglo at $2.8 million, making it the most expensive drug in the US. Given the lifetime benefits, it is likely that Zynteglo still meets commonly accepted cost-effectiveness thresholds using traditional methods.   Considering the cost-effectiveness information provided and the clinical data reported in the press release, please propose an outcomes-based contract for Zynteglo. Explain how the contract would work (outcomes, payments, timing, etc.). Describe which stakeholders would benefit from the contract and which would not. Discuss challenges that would need to be addressed – and recommend solutions when possible.   Finally, consider the two approaches used by ICER – (1) all lifetime savings costs are credited to the price of treatment, and (2) half of the lifetime cost savings are credited to society. What are the impacts on stakeholders of using approach #2? The concept of crediting costs to society fairly new, given the advent of curative therapies. As such, it is not a widely accepted or standardized approach. Do you think this approach should be adopted? Why or why not?   Your response should be 500 to 750 words (1 to 1 ½ pages) in length.
