A 60-yeаr-оld mаn presents fоr evаluatiоn of an ongoing tremor. He notes the tremor has been present for roughly 1 year. He reports the tremor occurs in both hands primarily with activity, most notably with writing and eating, and that it appears to improve with alcohol intake. He has a past medical history of well-controlled hypertension, hyperlipidemia, and benign prostatic hypertrophy. He reports his father had a similar tremor. On physical examination, his vital signs are normal, and a rhythmic, moderate amplitude tremor is noted to the wrist and fingers when his hand is outstretched and with finger-nose testing. The tremor is absent at rest. There is no dysmetria with finger-nose testing, his gait is normal, and there is no rigidity with passive movements of either arm.
