He reports gaining 5 pounds in a couple of days.  He reports his appetite is "normal."  He denies chest pain and is anxious about his condition.  KNDA Medications patient takes at home: Enalapril 20 mg by mouth twice a day Hydrochlorothiazide 25 mg by mouth twice a day Assessment: He is alert and oriented x4, HR regular 102 bpm, and +3 pitting pedal edema, respiratory rate unlabored, Crackles to the bases, Abdomen soft round non-tender positive bowel sounds x4, skin warm, dry, and intact.  Patient with frequent clear yellow urination and denies pain.   Vital Signs: Temp 98.2 F, Pulse 102, RR 22, BP 150/98  O2 sat 91% on Room air Weight 220 pounds BUN 10 mg/dL Creatine 1.5 mg/dL Hemoglobin 12.9 dL Sodium 135 mEq/L Potassium 2.8 mEq/L Chloride 95 mEq/L What should the nurse include in the teaching plan for the client?
