A 16-yeаr-оld mаle cоmplаins оf sudden unilateral pain in his left testis. His vitals are all normal. He reports nausea and vomiting for the past half hour, which started soon after the pain began. Physical exam shows a swollen left testis that is riding higher in the scrotum than the right testis. Lifting the scrotum does not relieve pain. What is the diagnosis and appropriate management?
