20-yeаr-оld Mаrcus hаs recently drоpped оut of college due to a sudden decline in his ability to function. Over the past three months, his roommates noticed him staying in his room for long periods, often whispering to himself or shouting at seemingly no one. He has told his sister that a man named “Rynor” visits him in the night to warn him about invisible enemies watching his every move. During conversations, Marcus jumps from topic to topic without logical connections, saying things like, “The moon is watching, but I can smell green. I must collect the bells for Rynor before the ceiling falls.” He has also invented unusual words like “carnimix” and “flebberating” in his speech. When asked about his day, he frequently responds with rhyming phrases such as, “Fine and shine, grapevine, align.” His face appears blank most of the time, and he rarely initiates or maintains conversation with others. He often starts projects, like painting or writing, but never finishes them. Marcus has also been observed making repetitive hand movements and refusing to eat, claiming the food has been “encoded with messages.” After beginning treatment with antipsychotic medication, most of Marcus’s symptoms resolved within four months. Which diagnosis best fits Marcus's symptoms?
