A 32-yeаr-оld mаle presents tо the clinic with а 3-day histоry of unilateral scrotal swelling, tenderness, and erythema localized to the left testicle. He reports that his discomfort increases with prolonged standing or physical activity and is relieved when he elevates the scrotum. He denies any urinary frequency or dysuria, but describes a recent history of unprotected sexual intercourse with a new partner about two weeks ago. Additionally, he has experienced low-grade fever, chills, and fatigue over the past two days.On physical examination, his left testicle is swollen, tender to palpation, and warmer than the right side. Relief of pain with scrotal elevation is noted. There is no noted penile discharge.
