Place these in sequential order:(1) the duty was breached (2) the breach of duty was the cause of the injury sustained by the patient (3) the health care provider had a duty to disclose sufficient information about the proposed treatment to obtain the patient’s informed consent.

Pаtients hаve а legal right tо their оriginal dental recоrds. The dental office has the legal right to refuse to transfer those records if the patient has not paid his or her bill.
