74-yeаr-оld mаle is seen in physiciаns' оffice fоr ongoing care and mentions he is concerned about his breathing when outdoors. His ability to walk around his neighborhood is diminished over the past several months and he now frequently has to stop and “catch his breath”. He denies chest pain but thinks that the shortness of breath is rapidly getting worse. He has not in any recent upper respiratory tract illnesses. He has an 80-pack year smoking history. Vitals: HR: 96 bpm; RR: 16; BP: 148 / 84, & T: 98.6; O2 Sat: 91% (RA) Examination: He's noted to be thin but with a wide chest. He is breathing mostly through his mouth and using accessory muscles of respiration. His heart sounds are distant but otherwise normal.
