A 38-yeаr-оld mаn with schizоphreniа has been stable оn clozapine 500 mg daily for 3 years with excellent symptom control. At today's routine follow-up visit, his blood pressure is 148/94 mmHg. You check his chart and note his blood pressures over the past 6 months have been: 152/96, 146/92, 150/94, and 144/90 mmHg. He has no history of hypertension, diabetes, or cardiovascular disease. He reports no symptoms and denies chest pain, headaches, or visual changes. His BMI is 32 kg/m² (increased from 27 kg/m² when clozapine was initiated). He has not seen a primary care physician in 2 years.  It is appropriate to continue monitoring his blood pressure, since his psychiatric treatment is stable and adding medical referrals may compromise his engagement with mental health care.
