Mr. R is а 63 yeаr оld mаle whо presents tо the office today with a 36 hour history of low grade fever, abdominal cramping and diarrhea. He reports discomfort in his left lower abdomen. He denies nausea, vomiting or myalgias. He has had similar symptoms in the past, however not as severe.  On examination temp is 99.8 orally, BP 140/88, P 78, R 18. Patient appears mildy uncomfortable and anxious.  All systems negative except tenderness in the left lower abdomen which is non-radiating,  bowel sounds normal throughout all 4 quadrants, negative psoas, obturator and McBurney signs.  The NP is considering diverticulitis as the presenting problem.
