KOHLMAN EVALUATION OF LIVING SKILLS   Mrs. T is аn 82 yeаr оld femаle whо was recently hоspitalized with a mild CVA (stroke).  Prior to this event, she lived alone in an assisted living apartment.  She received assistance with weekly housekeeping and one meal a day.  For an additional fee, she is able to receive up to two meals a day and assistance with showering. In order to return to her apartment, she needs to be independent with: basic activities of daily living, light meal preparation, walking to get around her apartment, and getting to/from the dining room.  She will be discharged home with a quad cane.  Her son and his family live in the same town and provide intermittent assistance, especially with the management of her finances and bills.  Mrs. T would like to return to her apartment.  The Kohlman Evaluation of Living Skills (KELS) was used to evaluate Mrs. T’s ability to return to her assisted living apartment as she is nearing the end of her inpatient subacute rehabilitation stay.  Refer to the kels score.pdf form to help follow along with the following results:  Self Care Frequency of self-care activities (self-report)- Independent T’s self-care routine (per her report) is appropriate according to the KELS criteria Appearance- Independent She reports independence with dressing, taking care of her “teeth”, combing her hair, and hand/face washing. Safety and Health Awareness of dangerous household situations (photographs)- Needs Assistance Verbal prompting was required for Mrs. T to recognize unsafe situations in the pictures, esp. those pictures with trip hazards and cords in the walkway. Identification of appropriate action for sickness and accidents- Independent T was able to identify the need to tell a nurse at the facility if she wasn’t feeling well but wasn’t able to come up with an appropriate plan for a more serious healthcare crisis or an emergency Knowledge of location of medical and dental facilities- Needs Assistance Money Management Use of money in purchasing items- Independent T is able to make simple purchases at the store within the assisted living facility with the cash provided by her son Obtain and maintain source of income- Needs Assistance T’s son provides assistance with all finances Budgeting of money for food- Needs Assistance T’s son manages her monthly budget Budgeting of monthly income- Needs Assistance T’s son manages her monthly budget Use of banking forms- Needs Assistance T’s son manages her banking Payment of bills- Needs Assistance T’s son provides assistance with paying of bills Transportation and Telephone Mobility within community- Needs Assistance Community mobility and transportation is provided by her son or the assisted living facility Basic knowledge of transit system- Not applicable Use of phone book and telephone- Independent T can independently use her landline telephone and a telephone book.
