CASE STUDY:  Phyllis Vаnce Phyllis, а 78-yeаr-оld female diagnоsed with pyelоnephritis, has been on the Renal Medical-Surgical unit for 3 days receiving treatment for pyelonephritis - including intravenous fluids and ceftriaxone (Rocephin), an intravenous cephalosporin (antibiotic). The previous nurse reports that Phyllis did not void overnight and has requested that the unlicensed assistive personnel (UAP) assist her to the commode immediately upon waking. Per the chart, the patient's last urine output was 200 mL 14 hours ago. The nurse enters the room just after the UAP assists Phyllis back to bed from the commode. Morning assessment reveals the following: Vital signs: Temperature 98.8 F (37.1◦C ), blood pressure 100/50 mm Hg, pulse 110 bpm, respirations 24 breaths per minute, oxygen saturation 95% on RA. Neuro: Oriented to self only. Awake and alert. Daughter is at bedside. Genitourinary: Complains of 3/10 flank pain.  125 mL of brown, foul-smelling urine in bedside commode.
