CASE STUDY: NOAH Nоаh is а 3-mоnth-оld infаnt diagnosed with Shaken Baby Syndrome. When his parents arrived with him to the Emergency Room, they stated that he wasn't acting right when they picked him up from daycare. He was crying profusely and nothing his mom did would help to soothe him. The daycare provider, Miss Marlene, stated that Noah had fallen off of the bed. The doctors explained to Noah's parents that the bruising on his body and damage to his brain indicates he was shaken. According to scans, the trauma from being shaken resulted in diffuse brain swelling. He now exhibits spastic tone in bilateral upper extremities. He also has a Cortical Vision Impairment (CVI) due to the damage in the brain and has had intermittent seizures since the incident. You received orders to evaluate and treat once Noah has stabilized and return home 8 weeks later. He is now 5 months old.

  Yоu аre dоing experiments оn two sets of monkeys who аre trаined to saccade to targets. The targets are arranged in a diamond like pattern as illustrated above, with the center of the diamonds being directly in front of the animal's head. The targets are activated sequentially, so that the animals trace a diamond path with their eyes. On some sets of trials, the animals move their eyes in a sequence from a->b->c->d->a and in other sets of trials from A->B->C->D->A. One group of monkeys has been raised with prisms in which objects appear to be 10 deg to right, and one has been raised normally. You conduct all of these experiments when they are adults and with the prisms removed. 1. First you conduct experiments with visual targets. Where would the two groups of animals saccade to? Which positions or saccades would be affected? Then you repeat the experiments with auditory targets at the same positions in the dark. Again describe any differences between the two groups of animals. 2. In normally raised animals, you record neurons from a midbrain structure which contains both visual, multimodal (visual/auditory) and motor neurons related to eye movements. In this structure you find a cell whose discharge rate is strongest immediately prior to saccades from a to b.  What is the name of this structure and on which side of the brain is it? Where would the cell be in terms of depth within the structure? 3. You then locally inactivate the structure using a pipette attached to the electrode recording the a->b cell. For the two diamonds (a->b->c->d and A->B->C->D) what saccades would be affected and how? What if the animals do the sequence in reverse (d->c->b->a and D->C->B->A)?
