The questiоns belоw аre intended tо simulаte the informаtion that you would provide when documenting Medication Therapy Management (MTM) during a Comprehensive Medication Review (CMR). This CMR occurred in the ambulatory care setting. You can assume that immediately after your patient encounter, you talked with the patient's PCP and received approval for ALL changes you recommend before your patient left the office. Therefore, the PMR should reflect the changes you recommended.

Failure to do so could result in an unsatisfactory performance on this station. You do NOT need to include the priority (high/medium/low), and you do NOT need to list them in order of priority. This document is intended to go to the patient's prescriber immediately AFTER the visit (i.e., these recommendations will be sent to the prescriber after the patient leaves the visit with you) You are only responsible for select immunizations for CMR activities (see recorded content for details).  Do not worry about formatting - focus on the content of your answer
