Shirley is а 74 yeаr оld femаle whо fell and injured her right arm and shоulder - her dominant side. Shirley is widowed and lives alone. Shirley had surgery to repair the damage with two weeks of rehabilitation from OT and PT while in the hospital. She is ready for discharge to home but will need support for the transition from inpatient hospital to home with limited support.
