TPA is administered however the client continues to have neurological deficits. A CTA is completed and shows a Large Vessel Occlusion (LVO) in the Left Middle Cerebral Artery (MCA). The client is taken to the cath lab for a mechanical thrombectomy. Access is initiated through the right femoral artery and the clot is successfully removed. Following a successful thrombectomy, the patient is moved to the Neuro-ICU. What is the priority nursing assessment for the first 2 hours?
