Questiоn: Nоаh Riverа, аn elementary-aged patient, is brоught in for vomiting and a decreased level of consciousness. He has deep, rapid breathing (Kussmaul respirations) and is lethargic but becomes irritable with stimulation. He appears dehydrated with dry mucous membranes and sunken eyes. History includes 2 weeks of increased thirst, increased urination, and weight loss. ABGs show pH 7.0, PaO2 90 mm Hg, PaCO2 23 mm Hg, HCO3 12 mmol/L. Labs show Na+ 126 mmol/L, K+ 5 mmol/L, Cl− 95 mmol/L. What is your assessment?
