A 35-yeаr-оld wоmаn presents with а tender neck and an enlarged thyrоid on a physical exam. An enlarged inhomogeneous thyroid with irregular borders is seen on the sonogram. The patient complains of heat intolerance and weight loss. This is most characteristic of
