(16 Points) Student: Justin  |  Age: 10 years  |  Setting: Elementary school Diagnoses: Down syndrome; mild intellectual disability; progressive visual disorder Occupational Profile Justin is a 10-year-old student with Down syndrome, mild intellectual disability, and a progressive visual disorder that is expected to cause gradual vision loss as he grows older. He is expected to maintain vision throughout his school career. Justin receives services from a vision consultant and school-based occupational therapy to support access to educational materials, handwriting, and participation in classroom routines. Justin uses a closed-circuit television (CCTV) magnification system in the classroom. He positions books or worksheets under its camera so enlarged print appears on a computer screen at his desk. This accommodation supports access to near-point printed materials. His independence with device setup, adjustment, and use during assignments has not yet been reported. Current OT priorities include writing his first and last name in cursive, forming lowercase cursive letters correctly, copying a sentence in cursive, and remembering his mother’s cell phone number for functional safety. Justin’s handwriting is less legible at the word level than at the letter level, with spacing errors noted in manuscript writing. These difficulties may interfere with the readability of schoolwork and efficient written communication. Justin may benefit from structured instruction, repeated opportunities to practice functional writing, and visual adaptations matched to his needs. The specific effects of visual-motor coordination, fine motor control, cognition, and visual fatigue on his handwriting require further observation rather than assumption. Collaboration among the OT practitioner, vision consultant, teacher, and family can support consistent accommodations and generalization of skills. Occupational Therapy Evaluation Results Evaluation Tool of Children’s Handwriting (ETCH) The ETCH was administered to establish a baseline of manuscript and cursive handwriting legibility. Assistive Technology & Environmental Considerations Assistive technology and classroom participation: Justin requires assistance with positioning school tools and materials, adjusting CCTV magnification, switching between reading and writing, and completing classroom assignments with accommodations. OT Evaluation Summary ETCH results document reduced word legibility in both manuscript and cursive writing (60% each), with spacing errors identified in manuscript. Justin's progressive visual impairment and use of CCTV are important contextual factors for school-based intervention planning. Further direct observation is recommended to identify contributing performance factors and determine effective supports. The goals below target functional cursive writing; remembering his mother’s phone number remains an additional OT priority not measured by the ETCH. Long-Term Goal — End of Academic Year By the end of the academic year, Justin will demonstrate improved functional handwriting skills by independently writing his first and last name in cursive and copying a 5–7 word sentence in cursive with at least 85% letter legibility and 80% word legibility, utilizing CCTV magnification and/or adapted writing materials as needed, in 4 out of 5 opportunities, as measured by handwriting samples and OT documentation. Short-Term Goal 1 — End of First Semester By the end of the first semester, Justin will correctly form lowercase cursive letters and write his first and last name in cursive with at least 80% letter legibility, using CCTV magnification and/or adapted writing materials as needed, with no more than 2 verbal cues in 4 out of 5 opportunities.
