A pаssive pаtient оn cоnstаnt-flоw volume control has an exhaled VT of 500 mL, inspiratory flow of 60 L/min, and total PEEP of 5 cm H2O. PIP increases from 28 to 40 cm H2O while Pplat remains 20 cm H2O. Diffuse wheezing is present. After prescribed bronchodilator treatment, PIP falls to 30 cm H2O; Pplat and total PEEP are unchanged. There is no leak. Calculate static compliance before treatment using the current Pplat. Calculate airway resistance immediately before and after treatment. Explain what the pressure pattern and treatment response suggest. Name two additional causes of this pressure pattern that should be assessed if the problem persists.
