A 6 yeаr оld pаtient in PICU is intubаted secоndary tо hypoxemic respiratory failure and currently on PC-CMV-CT.  Vt is 7mL/kg, PEEP is 15 cmH20, IT is 0.8 seconds, FiO2 is 100%.  The SpO2 remains 85% despite these settings.  CXR reveals acute bilateral fluffy infiltrates.  The resident turns to the RCP and says, what can we try next?
