He has a PMH history for colon cancer, COPD, hypertension, and hyperlipidemia.  The pain is achy in nature & radiates to right buttocks. He has urinary incontinence that he relates to being old.  No bowel incontinence. No known injury or trauma.  He was recently on prednisone for 5 days for COPD exacerbation, but his last exacerbation was several years ago.  What would be considered a red flag of this patient?
