He has suicidal thoughts, has lost 30 pounds, indicates that he is not sleeping much at night, and has no interest in sex or any other pleasurable activity.  When asked if he has ever had a period of extreme elation, extreme activity, and extreme acting out behavior, he says no. However, he does indicate, “I have had several periods of high productivity. These were noticeably different from depression and other times. I slept very little during those times and yet I felt energized. I worked late and when I came home, I was still energetic and accomplished things around the house. I was a little high when I went thru these times. I never had any problems during those times. These energy periods did not last very long usually about four days.
