He has had diabetes for the past 5 years and has been taking Glucophage 1000mg twice a day for his diabetes and losartan 25mg for his blood pressure. You have not seen him in the past 2 years. His most recent A1c is 8.3 and his labs show proteinuria with chronic kidney disease (GFR 40).  What would be the best treatment plan for his diabetes?
