SpO2 checked, now 95% on 2L/min O2 via NC.    8/20/23 at 0945:   Laboratory Values  Result  Reference Range  White blood cells (WBCs)  15.2 103/mm3  4-10 103/mm3  Red blood cells (RBCs)  3.59 cells/L  4.2-5.9 cells/L  Potassium (K+)  4.8 mEq/L  3.5-5.0 mEq/L  Glucose (FBS/BS)  314 mg/dL  70-100 mg/dL    8/20/23 at 1000: Client with pierced lip breathing, repositioning self on bed for optimal breathing. Increased complaints of dyspnea. Lung sounds with expiratory wheezing, clear to upper lobes, faint crackles to bilateral posterior base lobes. SpO2 89% on 2L/min O2 via NC.  The nurse has received orders from the provider.
