Shоrt Answer Cаse Study (Cumulаtive + Unit 4)  A 51-yeаr-оld male has a 12-year histоry of human immunodeficiency virus (HIV) and hepatitis C virus (HCV), which he contracted following years of intravenous drug use. Approximately 4 weeks ago, the patient complained of a persistent headache, nasal congestion, blurred vision, and a nonproductive cough, and blood tests revealed slightly elevated blood glucose and low blood albumin levels. He was referred to the nephrology clinic at the Massachusetts General Hospital.   Upon admission, the patient reported an increase in urinary frequency, and he described his urine as foamy. His blood pressure was 130/90 mm Hg, pulse 88 beats per minute, and weight 236 lbs. Breath sounds were decreased, oxygen saturation was 93% while breathing ambient air, there was evidence of pulmonary edema, and supplemental oxygen was administered. A grade 3/6 heart murmur due to insufficiency in the right atrioventricular (AV) valve was audible at the apex of the heart. There was 2+ edema of the knees. The patient had a history of chronic night sweats, headaches, joint stiffness (for which he took non-steroidal anti-inflammatories - NSAIDs), coronary artery disease with a history of myocardial infarction 6 years prior, hyperlipidemia, obesity, depression, gout, and glucose intolerance. He drank alcohol and smoked cigars occasionally. He reported no intravenous drug use since his HIV and HCV diagnosis. Current medications include amlodipine (Ca2+ channel inhibitor), antiretroviral medications, lisinopril (ACE inhibitor), metoprolol (β1-adrenergic antagonist), pantoprazole (proton-pump inhibitor) and NSAIDs. Doctors administered furosemide (Na-K-2Cl co-transporter inhibitor) and acetaminophen for pain.   Ten weeks later, the patient’s edema significantly improved, yet urine was still foamy. Blood glucose levels were elevated, and urine albumin levels had risen significantly. Eight weeks later, his blood urea levels continued to rise, glomerular filtration rate (GFR) dropped significantly, and blood glucose levels remained elevated. His blood pressure was 125/80 mm Hg, pulse 100 beats per minute, and weight 249 lbs. Doctors performed a differential diagnosis to determine the cause of apparent nephrotic syndrome.   Nephrotic syndrome is common in patients with either HIV or HCV. A core biopsy of the renal cortex confirmed fibrillary glomerulonephritis associated with HCV. Doctors treated with immunosuppressive drugs to protect the kidneys, followed by antiviral therapy for HCV. The drugs included cyclosporine (cytokine inhibitor) followed by peginterferon (interferon), and ribavirin (antiviral). Unfortunately, the patient began experiencing progressive anemia with possible myocardial infarction and severe acidosis. Treatment with ribavirin and peginterferon were discontinued, and the patient died soon thereafter.   Adapted from Case 5-2010.
