Operаtive Repоrt Preоperаtive Diаgnоsis:  Benign Prostatic Hyperplasia Postoperative Diagnosis: Benign Prostatic Hyperplasia Operation:  Transurethral resection of the prostate History:  65 year old man who has urinary obstruction and frequency.  The risks and benefits of the procedure were explained in full to the patient and he agreed to have a transurethral resection of the prostate. Description of Procedure: Spinal anesthesia was given pre-operatively to the patient.  Prepped and draped in usual fashion.  A pancytoscopy showed the presence of orthotopic ureteral orifices with a large median lobe and large kissing lateral lobes.  The prostate was short in length with the veru identified.   Using the rectoscope and an electrocautery knife, I resected the lateral lobes and the floor of the prostate.  I did this systematically, maintaining good hemostasis throughout.  A small amount of tissue anteriorly was also resected.  Tissue resected proximal to the veru until a nice clear channel was present.  Hemostasis was maintained throughout using coagulation.  Once hemostasis was confirmed, I rechecked the bladder.  The ureteral orifices were seen.  No bleeding was seen and the channel was clear.  Then a 22 French three-way Foley Catheter was placed, and a 30-cubic cm balloon was filled.  Continuous bladder irrigation was then set up.  Patient was taken to recovery in good condition. 1.
