A 58 yeаr оld presents frоm the diаlysis clinic with hypоtension аnd syncope. Initial blood pressure 74/51 at the dialysis clinic. Emergency medical services administered a 500cc bolus of normal saline prior to arrival in the emergency department (ED). Blood pressure slightly improved to 81/59. The patient is alert, oriented, and following all commands. He has a history of hypertension, diabetes, ESRD, COPD, and sleep apnea. He reports a mild dull chest pain and occasional shortness of breath. Initial lab work is within normal limits except for Creatinine (7.8) and BUN (95). ESR and CRP are normal. Cardiac enzymes are normal.  Chest x-ray showing moderate cardiomegaly. EKG shows sinus rhythm with a right bundle branch block.  The AGACNP hospitalist admits the patient to the cardiac floor and orders an echocardiogram which estimates a pericardial effusion of 400cc.  What is the most likely cause of the pericardial effusion?
