Agnes was told to wear her sling continuously for 2 more weeks, except for exercise and self-care, then discontinue it completely by 4 weeks post-op.    The orthopedist saw her yesterday for a post-op check-up and suture removal.  He ordered physical therapy to begin for “AAROM and gentle PROM, progress as tolerated”.   Below is information from the initial PT evaluation completed today at 2-weeks post-op:    PROM right shoulder (supine):    Flexion 50 degrees                        Abduction 40 degreesExternal rotation 30 degrees (measured with elbow at side)    AROM right elbow:    10 - 110 degrees    PT Plan:  See in clinic 2 times/week starting with PROM/AAROM to the shoulder progressing to AROM & submaximal isometrics in 2 weeks. 41.    At 8-weeks post-op, you notice Agnes is still having great difficulty with reaching for her seat belt, AROM is still limited; and you note excessive shoulder shrugging when attempting this task.  You have also noted that she is holding her right arm at her side when walking as compared to the left arm.  In order to assist her with both of the problems above, the most appropriate exercises would be:a.    Resisted flexion with green tubing to shoulder height; shoulder retraction. b.    Scapular stabilization exercise; manually resisted PNF diagonals.c.    PNF diagonals with resistance; push-upsd.    Scapular stabilization; standing flexion with 3 lb. weights.
