Case Study: A 87 year-old client is hospitalized with metastatic small cell lung cancer that has spread to her bones. She was transferred from a skilled nursing facility after fracturing her right humerus in a fall. Her diagnoses include chronic obstructive pulmonary disease (COPD), Type 2 Diabetes, Osteoporosis, Hypercholesterolemia, and Hypertension. She has not improved with physical therapy, and for the last two weeks, she reports fatigue and anorexia. The client is experiencing diminishing breath sounds, increasing dyspnea, generalized weakness, stage 2 and stage 4 pressure injuries, and severe pain (9/10) in her right arm.  Vital Signs   Time On admission After Morphine Temp 99.2 99.2 P 110 62 RR 28 16 B/P 150/90 98/70 Pulse oximeter 89% on 2L 92% on 4L Medications   Morphine 4 mg IV q 6 hours Codeine 30 mg one po every 4 hours Tylenol 650 mg one to two po every 4 hours The client’s labs indicate poorly controlled Diabetes (glucose 259, HgbA1C is 9.2%). Order for Oxygen per nasal canula to keep O2 saturation above 92%. After discussing care goals, the client and her family choose hospice. She is now comfortable on Morphine (Pain 1/10), sleeping much of the time, with decreased eating and non-labored breathing. She appears peaceful; vital signs have decreased further, see (after medication). Her family expresses concern about her low oral intake.
