R.F. is a 74-year-old long term care resident. He is 5’11” tall, is large-framed, and weighs 146#. As the RDN you have been tracking his weight for the past six months. Six months ago R.F. weighed 172#. LTC staff report poor po intake at breakfast, fair intake at lunch, and that the resident often refuses his dinner meal. You have met with R.F. to discuss his dietary likes and dislikes and the dietary dept. has been making requested accommodations in an effort to improve R.F.’s po intake. You previously recommended the addition of nutritional supplements but R.F. refused them. At your request, R.F. is going to the dining hall for all meals. Although he does not typically eat his dinner meal, he will go and sit with other residents and socialize while they eat their dinner. R.F. does have a diagnosis of early onset dementia and has recently developed a Stage II pressure ulcer on his buttocks. He has no known family that visits.  Last Friday during a weekly quality control meeting, you and the team discussed the possibility of beginning enteral nutrition. You arrived at work today (Monday) and see a physician order for a recommendation to start enteral nutrition.
