Client remained stable throughout procedure; no major complications noted. New hardware placed and incision to right hip with sterile dressing applied by surgeon. Blood loss noted and 1 unit of packed red blood cells transfused during surgery. Dose of antibiotic given IV right before anesthesia for infection prevention. 1005Temp. 96.4F, HR 88 BPM, Resp 24 breaths/min, SpO2 90%, BP 106/56. Responding to verbal and tactile stimuli. Oxygen on at 4L. Continuous O2 saturation monitored. Jaw thrust maneuver performed for snoring sounds and decreased level of consciousness. Placed on left side and HOB elevated 30 degrees, resp unlabored and even. Dressing intact to right hip with bloody drainage. 1020 Temp 97.4F, HR 92 slightly irregular, resp 22, SpO2 90%, BP 100/56. Client still lethargic but arousing and answering yes/no questions. Denies any current pain. O2 at 6 L. More blood noted on right hip bandage. Skin color pale and cool to touch.
