The client, Mr. Harris, a 26-year-old man, is in the emergency room with his girlfriend who states, “I don’t know what is happening. He has been up for 10 days. Ted painted the garage and the utility room. He started to rewire the living room and has built some shelves for the kitchen. He never slept much but this is ridiculous. He wants to go on one of those reality shows and be the carpenter guy. But Ted is no carpenter or any other kind of “fix it” kind of person. He is an accountant not a handy man! He has all his savings in those carry-alls around his waist. I am scared! What is happening?” The client states, “I am going to be like the guys with their home shows. I’ve got it all written down here. . . Let me read it to you! . . I’ve never been so enthusiastic or full of energy. . . Still can’t sleep, been up for three days, but not tired.” He springs up and paces the room talking obsessively. After a drug screen with no evidence of substances use, you tentatively diagnose of what?

Penny, 23, indicates that she has suffered from these symptoms at least seven times in the past three years when she has what she calls her, episodes. “When I get this way I am depressed every day and all day. There is nothing I enjoy at all, even stuff I always have fun doing. I lose weight, because there is nothing that I want to eat. I just drink water and eat ice, but it is because everything is unappealing. I can’t sleep and wander around the house cleaning things that I know are clean. But during the day, I just can’t get up. I feel so very heavy, like there are weights on my feet. I am so tired and fatigued when these things come on me. I also have such a feeling of worthlessness and guilt, like I am responsible for everything in the world that is wrong. I can’t concentrate or come to conclusions. And although I would never kill myself, I sometimes wonder why I go on when I have these episodes. It seems so pointless.” Penny meets criteria for what?

Mary, a person with depression indicates that her life is devoid of pleasure and positive emotion. She states, “I just don’t enjoy anything anymore. I always had a wonderful time when Todd and I went on vacations to interesting places. We went to Amsterdam last month and it was so colorless and drab. I loved being out in nature, riding my bike and surfing. I loved my home and just sitting in the kitchen watching the birds from my kitchen table gave me such pleasure. Reading a good novel or watching an excellent play were all sources of such fun for me. But now I find no delight in anything. The pleasure has just been sucked out of my life.” Mary is describing what?
