As a student at Wallace State Community College, I understand that I will take some tests/exams remotely. With respect to any tests/exams I take remotely, I do hereby voluntarily and under no duress, consent to the following: The addition of a Chrome extension for outside proctoring service. A scan of the computer I will use to take the test/exam for open apps, tabs, hardware, devices, etc. Allowing remote access by the proctor to the computer I will use to take the test/exam to address security and/or technical issues. Taking a photo of myself. Displaying a valid photo ID of myself. Allowing a video scan of the room I will take the test/exam in and a scan of the testing surface/desk. Being video and audio recorded while taking the test/exam by way of the webcam on the computer I will use to take the test/exam. Having the screen on the computer I will use to take the test/exam recorded during the test/exam. The recording of the tests/exams and related information being retained by the online testing proctor and Wallace State Community college having access to such recording(s) and related information. This consent shall remain in full force and effect until such time as I submit a written notice of the withdrawal of my consent to Wallace State Community College’s Office of Admissions.
