She lives at home with her parents and two older brothers who adore her and are quick to do things for her. Nyah received Botox injections 3 weeks ago with a goal of improving gross motor control. Decreased muscle tone has been noted overall. She has a gait trainer that is used only with adult assistance to avoid injury. Parents are asking that wheelchair fit consultation be a primary consideration of your evaluation. Although her parents’ initial focus was on her mobility skills, they have noticed she lacks control of her hands and arms. For instance, she has difficulty placing toys (e.g., completing shape sorter), drawing with a crayon, or participating in self-care tasks. Nyah can finger-feed herself larger pieces of food. She is observed to frequently choke, spit, or sputter when she drinks water or juice. Nyah is toilet trained but needs help with transfers on and off of the toilet. She tends to lean to her left side and has difficulty getting up and down from the toilet. Nyah’s parents primarily dress her. She assists by extending her unaffected arm and leg to thread them. Safety is an issue at bath time, as she has outgrown infant seats and lacks independent stability for sitting.
