A 44 year old woman presents to your office in the afternoon with complaints of abdominal pain, rating the pain as an 8/10 on pain scale. She states the pain started abruptly approximately 6 hours ago and is mainly in her right upper quadrant and right shoulder. She ate a sausage and cheese casserole early this morning but, has vomited several times over the past two hours. She appears in acute distress and is lying very still on the exam table. VS= Temperature- 100.4º F; BP- 124/84; Pulse- 110 bpm; Respirations- 20. On exam she has abdominal guarding and a positive Murphy’s sign. What is her most likely diagnosis?
