Client: Male, 82 years old  Diagnosis: Osteomyelitis of the left hip, not a surgical candidate Baseline mental status: Alert and oriented per family. Current Status: Day 5 of mechanical ventilation. Sedation (Propofol) was titrated down 2 hours ago for a spontaneous awakening trial (SAT). Nurse Assessment Findings: The client opens his eyes to verbal stimuli but cannot follow commands. When asked to “Squeeze my hand when I say the letter ‘A'” during a string of letters (S-A-V-E-A-H-A-A-R-T), the client squeezes randomly and misses several “A”s. The client’s RASS (Richmond Agitation-Sedation Scale) score is -1 (Drowsy). The nurse notes the client is picking at his sheets, is restless, and attempting to sit up despite being weak.   Using the above information, determine if the patient has delirium.
