Clinical Findings: Periodontal assessment reveals 4-6 mm pockets, moderate bleeding on probing, furcation involvement. She has limited ability to open her mouth due to TMD problems. Diagnosis: Generalized Stage III Periodontitis Grade B (no grade modifiers), Caries Controlled, Deposit 3 Supplemental Notes: All pockets 4 mm or greater require local anesthesia for root planing (pulpal and soft tissue). Periodontal therapy will begin today.
