Peggy Fender is well known to the emergency department (ED) staff. She’s a 59-year-old woman whose visit today is similar to her frequent visits in the past. She arrives in the afternoon, appearing intoxicated and complaining of abdominal pain, particularly in the right upper quadrant (RUQ) of her abdominopelvic cavity. Tim, the PA on call conducts a physical examination. Mrs. Fender appears emaciated with bruising of various ages on her arms, legs, and face. She is jaundiced (yellow-skinned) and has the strong smell of alcohol on her breath. Tim palpates the inferior border of her liver, which is hard and enlarged; Mrs. Fender moans with pain. Her entire abdomen is ascetic (third spacing). Blood tests are done and show elevated blood liver enzymes aminotransferases (ALT & AST), hyperglycemia, and prolonged prothrombin time (PT – slowed clotting time). Based on her history, and present findings, Tim diagnosis Mrs. Fender with alcoholic cirrhosis, which is the result of chronic inflammation of the liver from heavy, consistent alcohol ingestion. If alcohol abuse continues long-term, it leads to fat accumulation in the liver, followed by fibrosis and severe liver dysfunction.  1. Mrs. Fender is jaundiced. What is jaundice and why does it occur with liver disease? 2. What information is supplied by the increased AST and ALT blood levels - what does their presence in the blood indicate? 3. She is 5'3" tall and weighs 195 pounds. She is the mother of four children ranging in age from 10 - 18 years and has gained weight after each of her pregnancies. She works full-time and doesn't have much time for exercise. She wants to lose weight and isn't happy with her body (she has a 39" waist). She tries to eat well but eats fast food 2 - 3X per week and claims she "lives off of diet Coke". She has been feeling tired lately and made an appointment with her doctor because a coworker told her she might have a problem with her thyroid gland. Her physician completes a physical and orders blood work. Her heart rate and blood pressure are within the normal ranges. The blood work comes back with the following abnormalities: Serum triglyceride level: 200 mg/dL, HDL cholesterol: 48 mg/dL, fasting blood glucose: 130 mg/dL - her thyroid hormone levels are within normal limits. Her doctor tells her that he thinks she has "metabolic syndrome". He explains that this syndrome can increase her risk of type II diabetes or cardiovascular disease. She is told metabolic syndrome is becoming more common and that it has also been known as "syndrome X" or "insulin resistance syndrome". She decides to research this condition on the internet to share it with others.  1. How is metabolic syndrome diagnosed? (list 3 specific criteria for diagnosis) 2.
