As you kneel beside him, he gasps, "I... can't... breathe... can't... feel... any... thing."   INITIAL ASSESSMENT: Chief Complaint - Complaining of difficulty breathing and inability to feel anything Mental Status - Alert, verbally responsive, oriented x 4 Airway - Open and clear Breathing - Respirations labored at 24, with accessory muscle use noted in his neck; talking in one word sentences; lung sounds clear on the right and coarse on the left, in the mid-sternal area Circulation - Skin pale, warm, and dry; Radial pulse 50; BP 86/64   FOCUSED HISTORY: Events - He just received the motorcycle for his birthday and was taking it out for a ride when he entered the intersection and ran head-on into the automobile at a high rate of speed Previous Illness - Unknown Current health status - Unknown Allergies - NKDA Medications - Patient denies taking any medications on a regular basis   FOCUSED PHYSICAL: Current vital signs - BP 86/64, P 60, R 24 Other pertinent findings - His full-face helmet is intact with extensive scuffing on the left-side and top; a mid-sternal bruise about the size of a grapefruit is present; NO chest wall movement is noted, but regular rhythmic movement of the diaphragm is synchronized with his respirations; Bilateral lower-leg anterior lacerations extending down to the tibia, approximately 10 cm in length, are also noted, but are not actively bleeding; A laceration on the left lateral hip, extending posterior to the left buttock is bleeding profusely Ongoing care - Administration of 500 mL of normal saline resulted in vital signs of BP 100/72, P 58, and R 20.   Your ECG is shown below:
