S3 noted, diaphoretic Resp: Lungs crackles bi-lateral lower lobes Neuro: A&O x 4, feels weak and anxious GI: hypoactive BS x 4 quad., nauseous Renal: 20mL urine output since arrival WBC= 12,546 per mcL (Normal 5,000-11,000 per mcL) Platelets 200,000 (Normal 150,000-450,000 per mcL) Na+= 147 mEq/L (Normal 135-145) K+= 3.3 mEq/L (Normal 3.5-5.5)  Ca+= 9.8 mg/dL (Normal 8.6-10.2) Glucose= 181 mg/dL (Normal 70-110 mg/dL) HgBA1C= 5.8% (Normal 4-5.6%) BUN= 26  (Normal 6-24 mg/dL) Creatinine= 1.2 mg/dL (Normal 0.6-1.2 mg/dL) Troponin= 0.10 ng/mL (Normal 0-0.4 ng/mL) BNP= 48 (Normal 0.5-30 pg/mL) 1100 B/P= 110/78 mmHg Pulse= 98 Beats/min & Irregular Respirations= 20 Breaths/min SPO2= 96% on 5 liters nasal cannula (LNC) Pain=  1 (0-10 scale)   Cardiac: Normal Sinus Rhythm. S3 noted Resp: Faint Lungs crackles bi-lateral lower lobes Neuro: A&O x 4, states he is tired GI: hypoactive BS x 4 quad. Renal: Urine output 100mL/hour   WBC= 14,000 per mcL (Normal 5,000-11,000 per mcL) Platelets 175,000 (Normal 150,000-450,000 per mcL) Na+= 138 mEq/L (Normal 135-145) K+= 3.9 mEq/L (Normal 3.5-5.5)  Ca+= 9.7 mg/dL (Normal 8.6-10.2) Glucose=  128 mg/dL (Normal 70-110 mg/dL) HgBA1C= 5.8% (Normal 4-5.6%) BUN= 24  (Normal 6-24 mg/dL) Creatinine= 1.1 mg/dL (Normal 0.6-1.2 mg/dL) Troponin= 73.41 ng/mL (Normal 0-0.4 ng/mL) BNP= 127 (Normal 0.5-30 pg/mL)   1100: As a result of the rhythm identified in question 6 & 7, Mr. Jones was taken immediately to the Cath Lab.
