Anоrexiа cаrdiаc defect Patients with anоrexia will suffer atrоphy of their body tissues, including the heart muscle. While the heart muscle thins and atrophies, it pulls on the heart valve flaps, which are non-muscular and stay the same size. This allows for aberrant movement of the valve with resultant prolapse into the chambers. It can be heard when listening to the heart with a characteristic systolic murmur and a "click" sound. The left heart muscle is normally thicker as it pumps against the entire systemic circulation, and the effect will be more profound, so mitral valve prolapse (MVP) is more common. For the same reason, the other options (aortic regurgitation, pulmonary stenosis, mitral stenosis, tricuspid regurgitation) are incorrect. Anorexia is not known to cause any form of valvular stenosis, and the valve most affected will be the mitral valve. Recommended sleep hygiene measures include having regular bedtime and rise times, avoiding napping, limiting caffeine intake after lunch, limiting alcohol near bedtime, avoiding nicotine, going to bed only when feeling sleepy, avoiding exercise within 2 hours of bedtime, keeping the sleep environment quiet and dark, avoiding checking the time at night, and avoiding large meals near bedtime. Insomnia is considered short-term when it has lasted < 1 month, and it is considered chronic when persisting > 1 month. Short-term insomnia is usually due to physiologic stress and is often self-limited. However, patients with severe distress due to short-term insomnia can be offered pharmacologic treatment. The first-line treatment for chronic insomnia is cognitive behavioral therapy with or without concomitant short-term pharmacologic therapy. There are several pharmacologic treatment options, such as ramelteon (melatonin receptor agonist), melatonin, sedating antihistamines, nonbenzodiazepine benzodiazepine receptor agonists (zolpidem), trazodone, and suvorexant.
