Scenаriо: Enrоute  The Pаrаmedic is dispatched tо a residence for a 26-year-old patient pregnant female who suffered a seizure. The time of the call is 0700. The response time will be 6 minutes.   The Paramedic is partnered with an EMT, and a BLS fire department engine crew is dispatched with an ambulance. The patient is located inside the birthing center. It’s a clear spring afternoon, and the temperature is 88°F (31°C). A small hospital is located 10 minutes away, and the nearest comprehensive medical facility is 40 minutes away.  Scenario: Scene  The patient (90 kilograms) is supine on the floor altered in her bedroom. A family member states the patient has had severe headaches, vision, changes and swelling to lower extremities for the past few days. The patient has a history of 34-week gestation, gravida 5, parity 2, and 2 abortions. Prenatal care is up to date, with gestational diabetes and hypertension throughout the pregnancy. Patient’s blood pressure had been under control with exercise and food intake but recently was not compliant.  The patient has allergies to sulfa medications and iodine. The patient’s skin is normal, warm, and dry. Patient seems to be postictal; does not obey motor commands. Eyes are closed, and the pupils are 5 mm and reactive to light. The vital signs are BP 180/110, P 90, R 20, SpO2 92% on room air, and T 98.6°F (37°C). The blood glucose is 140 mg/dL.   Scene  What is the first-line medication for seizure prevention in this patient?
