The sciаtic nerve cоntаins brаnches frоm the L4 thrоugh S3 nerve roots and exits the pelvis through the greater sciatic foramen, inferior to the piriformis muscle. lt is vulnerable to injury during hip fractures and surgery because of its proximity to the hip joint. The sciatic nerve contains two main branches in the same sheath, the tibial and peroneal (fibular) nerves, which form two separate branches at the popliteal fossa. Most sciatic nerve injuries are neurapraxias, which result in focal demyelination but no death of axons or loss of continuity of the nerve. Sciatic nerve injuries may be incomplete, more often involving only the peroneal branch, but may involve both branches. The sciatic nerve does not supply sensation or motor fibers to muscles in the thigh. A complete sciatic nerve lesion results in loss of sensation in the lower leg except in the medial calf and foot, which may be spared due to innervation by the saphenous nerve, a branch of the femoral nerve. The peroneal and tibial nerves supply all compartments of the lower leg and foot, so all muscles, including the tibialis anterior, extensor hallucis longus and extensor digitorum longus, peroneus longus and brevis, gastrocnemius and soleus, tibialis posterior, and intrinsic muscles of the foot are affected. Knee extension and the knee jerk reflex are intact as the quadriceps are innervated by the femoral nerve. The ankle jerk reflex is absent. Diagnosis is made by physical examination. Observation is appropriate initial treatment, as neurapractic injuries will show improvement within a few weeks.
